Health care providers across Erie St. Clair are now working together to improve access to quality care.
Example: Getting to a medical appointment is important but it can sometimes be a challenge, especially for seniors. In order to make getting around easier, the ESC LHIN brought together five Windsor/Essex health transportation providers to improve transportation services. Now, arranging a ride is as easy as calling one number.
2) Improved Access and Wait Times:
The LHIN is focused on making the local health care system easier to access.
Example: For people with an addictions problem in Sarnia/Lambton, the closest withdrawal management programs were in London or Windsor. The ESC LHIN worked with Bluewater Health, local service agencies, and the community to create comprehensive local programs and services for people in withdrawal from drugs or alcohol. People suffering with substance use issues now receive care closer to home.
3) More Responsive to Local Needs:
Health care decisions are being made by people who live in the community and understand local needs.
Example: The ESC LHIN funds a program that places a new bilingual nurse practitioner in a community where many French-speaking seniors and other clients have difficulty communicating with their care providers. Understanding local needs allows us to improve access to care, which in turn helps seniors live at home and remain active in their communities.
4) Working with our Community:
Health Care providers and local residents are in close touch with the people who plan and manage their health care system.
Example: Approaching the end of a life is never easy, but knowing there are options can make a difficult time a little easier. Thanks to the efforts of local residents, health providers, and the ESC LHIN, people now have more options such as in-home palliative care and residential hospice programs. Sometimes better care is as simple as having a choice.
5) Better Value for Tax Dollars:
The ESC LHIN has challenged local health service providers to ensure that at least 85% of their funding is allocated to front-line care.
Example: Chatham-Kent Meals on Wheels is delivering more meals to more seniors than ever before. By reducing administrative costs, the organization is now able to deliver more healthy food – and friendly visits – to people in our communities.
