connecting South West Ontario (cSWO)

The cSWO Project is in the process of integrating healthcare systems acros the Erie St. Clair, Hamilton Niagara Haldimand Brant, South West and Waterloo Wellington LHINs. Building on the success and investments made in local, regional and provincial initiatives, cSWO is working closely with stakeholders to address specific local and regional needs to allow electronic patient health inforamtion to be seamlessly and securely shared. When it's complete, the project will be comprised of robust, scalable and re-usable building blocks capable of exchanging clincial data for better, timelier and more coordinated care, accelerating the delivery of electronic health records to the populace.

ClinicalConnect™, the regional clinical viewer

eHealth Ontario is leading the integration of electronic health care information across the province. Together with the local health integration networks (LHINs) and health service providers, eHealth Ontario is working to deliver integrated and coordinated health care for Ontarians. cSWO represents 30 per cent of Ontario’s population including the four south west LHINs – Erie St. Clair (ESC), South West (SW), Waterloo Wellington (WW) and Hamilton Niagara Haldimand Brant (HNHB). London Health Sciences Centre (LHSC) is the delivery partner for cSWO program implementation and management, accountable to eHealth Ontario. Hamilton Health Sciences Corporation (HHSC) is the key solution partner to cSWO delivering the integrated regional clinical viewer to clinicians in south west Ontario.
