TransForm joins LHIN in primary care tour

TransForm Shared Service Organization’s (TransForm) roots may be in Erie St. Clair’s hospitals, but it is branching out to primary care - thanks to a new primary care tour initiative launched by the Erie St. Clair Local Health Integration Network (ESC LHIN). TransForm is the cSWO Program’s Change Management and Adoption Delivery Partner (CM&A DP) in ESC.

“In the ESC LHIN, we have undertaken a shared effort to engage with primary care providers and organizations by linking the work of the primary care Council, LHIN, Community Care Access Centre, TransForm, chronic disease management, and mental health care so that we can collectively inform them about ongoing activities and provide them with more tools and mechanisms to support their efforts in caring for their patients and families,” said Dr. Martin Lees, chair of the ESC LHIN Primary Care Council.

“It has become increasingly clear that the foundation for improving quality and enhancing the integration of health care efforts is found in the primary care system and its providers … we also see the benefits of integrated health records systems and are looking forward to the connecting South West Ontario (cSWO) Program continuing to implement ehealth solutions to enhance these clinical goals.”

TransForm’s Chief Information Technology & Security Officer and eHealth Lead Paul Audet said he shares Dr. Lees’ enthusiasm for enhancing the integration of health care efforts and that the sessions he’s attended to date have been informative for all involved.

“We’re grateful for the opportunity to reach this important audience,” Paul said. “A big part of the cSWO Program’s process is listening to health care professionals to implement an integrated Electronic Health Record that provides the information they need to deliver safe, timely and top-quality health care.”

Funded by eHealth Ontario, the cSWO Program is the regional ehealth initiative implementing an integrated electronic health record (EHR) solution for all Ontarians.
