ClinicalConnect: ESC CCAC Client Notes Available
The Erie St. Clair Community Care Access Centre (ESC CCAC) has started sharing its client notes through cSWO Program’s Regional Clinical Viewer, further enhancing the value of ClinicalConnectTM to users across south west Ontario. Client notes are captured in the CCAC’s Client Health and Related Information System (CHRIS).
“Our CCAC is committed to providing quality and safe home and community care for our patients,” said Dave Richie, ESC CCAC Communications and Public Affairs Lead. “The use of ClinicalConnect is in-line with our strategic plan and is one of the avenues we use to ensure this high standard of care is delivered.”
Dave said the inclusion of client notes with the rest of the CCAC’s patient data was announced to ESC’s ClinicalConnect users in a January 19 memo.
“We are always interested in efficiencies such as ClinicalConnect that allow better communication throughout the health care sector in Erie St. Clair, resulting in consistent patient care,” he said.
The enhancement brings the ESC CCAC in line with best practices employed by the rest of the CCACs in south west Ontario. In addition to CCAC patient data, ClinicalConnect users can securely access patient records held in participating Hospital Information Systems (HISs) and oncology centres across south west Ontario along with data stored in the Ontario laboratories information system (OLIS), the Southwestern Ontario Diagnostic Imaging Network (SWODIN) and a nursing assessment repository.
The cSWO Program is part of eHealth Ontario’s initiative ConnectingOntario, which is enabling connectivity across the province to share critical medical data in a safe, secure digital format that will greatly improve patient outcomes while saving taxpayer money and improving efficiency across the system.
