Chatham-Kent Community Health Centres

Community Health Centres (CHCs) are non-profit organizations that provide primary care for individuals, families, and communities. Primary care is the first level of contact of individuals, the family and the community with the health system. Practically speaking it includes diagnosis, treatment, referral, counseling, education, monitoring and other activities related to health maintenance. CHCs use a community development approach to health care to identify and provide services to those who face access barriers such as language, poverty, disabilities or geography and those who have a higher risk of developing health problems.

Within the Erie St. Clair Local Health Integration Network (ESC LHIN) area, there are CHCs operating in Windsor/Essex County (Teen Health Centre with a Satellite at the Canadian Mental Health Association in Windsor and, the Sandwich Community Health Center), in Sarnia /Lambton County (North Lambton CHC in Forest with satellites in Watford and Sarnia) and in Grand Bend (Grand Bend Area Community Health Centre). This will be the first CHC for Chatham-Kent.
