Community Care Information Management Project
The Community Care Information Management (CCIM) Project is about improving patient outcomes and creating efficiencies in the health care system. CCIM is made up of two large groups - the CCAC and Long-Term Care Homes. CCIM also includes other organizations and programs such as Meals on Wheels, Alzheimer support, friendly visiting and transportation services.
The Board has approved the funding commitment of $11 million over four years to support the launch of the CCIM Project. This will allow our health service providers to purchase the necessary hardware and software as well as receive essential staff training. The outcomes of this project will significantly improve patient care by standardizing and improving the collection of data, as well as facilitating the coordination of services required by the consumers across multiple agencies. Additionally, it will provide valuable information to support better planning and funding of services.
The project represents our LHIN’s commitment to the provincial strategy and community support service agencies and their clients. The initial work of the project was funded directly by the Ministry of Health who derived funds from the re-allocations unspent program funds. LHINs across the province are now asked to take a greater role in administering and funding this new project.
CCIM is not eHealth, although it will ultimately be linked into eHealth's infrastructure. eHealth’s role is to create a comprehensive electronic health record that will bring together physicians, hospitals and CCIM through a seamless, linked-in, backend system. CCIM will allow us to continue to create measurable and sustainable health care for everyone across our LHIN.
