Integrated Health Service Plan (IHSP)

Introduction

This plan is driven by the decision of the Ministry of Health and Long-Term Care (MOHLTC) to make fundamental changes to the health care system in Ontario. The ministry recognizes that healthcare across the province needs to become more patient-focused, results-driven, integrated and sustainable. By establishing the Erie St. Clair Local Health Integration Network (LHIN), the province has given us the responsibility for local health transformation.

We will transform the local health care system by communicating with the community in several ways: local planning, coordination, integration, and delivery of key publicly funded health services.

Our Integrated Health Service Plan (IHSP) articulates eight strategic integration directions for the next three years. The purpose of this plan is to improve local health care outcomes for the citizens of our LHIN by offering opportunities for changes which will enhance health care delivery through the better integration of service planning.

Engagement and consultation with our local communities, health service providers and health service agencies played an important role in determining and confirming the strategic integration directions. In addition, we have made a determined effort to collect and analyze the most recent supporting population health and health planning data. We will continue to seek input about local health care needs through community consultation efforts and data analysis as we refine the Integrated Health Service Plan on an annual basis.
